Root canal calcification or obliteration (RCC) is commonly evidenced in anterior teeth produced by a pulp reaction mainly associated with trauma, caries and advanced age of the individual, where the root canal is totally or partially obliterated by deposition of mineral tissue. Endodontic treatment in cases of RCC is considered highly difficult and high risk, especially in anterior teeth, since dental tissue is greatly lost during location and communication with the root canal. The objective of this research was to carry out a systematic review on the effectiveness of endodontic treatment of calcified or obliterated anterior teeth using surgical guides for the preparation of the access preparation compared with the conventional technique. For this, the PRISMA protocol was used, where different databases such as PubMed, Scielo, EBSCO, Scopus were reviewed and to complement the following journals were added: Journal Of Endodontics and International Endodontics Journal, of the selected articles a total of 19 were included studies where the following data were extracted: type of technique to be used for the preparation of the endodontic access, location and communication of the root canal, amount of tooth tissue lost (mm) and risk of iatrogenesis (deviation in mm and angle). In conclusion, all the selected articles describe that the guided technique allows a preparation of the access cavity to the canal system with high precision rates and minimal loss of tooth structure compared to the conventional technique. New high-evidence studies are needed that a message New high-evidence studies are needed to consolidate the use of surgical guides in the endodontic access of calcified or obliterated teeth as a standard technique.en
